FIRST, THE BASICS:

A Notice to Vacate is also called a Notice to Quit. This “Notice” is a mechanism used by a landlord to evict a tenant who has violated the terms of a lease or rental agreement. The most common use of the Notice to Vacate ( Quit ) is when a tenant has not paid rent.
The Notice to Vacate ( Quit ) must include the tenant’s name and the address or description of the property, the reason for the demand, a date and the landlord’s name.

Collect the following information to complete your form:
* Effective ( DATE ) of the Notice
* Landlord / Owner’s ( Name )
* Tenant(s) ( Name )
* Address of ( Property )
* Amount of ( Days ) to Pay or Surrender Property
* Date Tenancy Commenced ( date moved in )
* Regular Amount of Rent: how paid ( daily, weekly, monthly, etc.)
* Amount of ( All Deposits ) breakdown ( security,cleaning,pet, etc.)
* Date … Rental Payment Became Delinquent
* Date … Rental Payment Became Delinquent
* Total Amount of ( Rent Due )
* Total Amount of other charges due Late charges, bad check, etc.
* Name and Phone Number of Person that Tenant must contact
* Name of Court where Legal Action will be brought
